flight log examples?

hello
i don't have a heli yet but i just wanna get as much info as a can before i start. i read in some places that you schould keep a detailed flight log for every flight. if it's not too much trouble, can someone please put up an example of what one should look like?

Things to record:

What heli, location you're flying at, flight time in minutes, what number(total) flight this is for this heli, fuel used or battery mah consumed, then if you want to get detailed whenever you crash you can put a cost in there for parts replacement.

You can also use www.fly-log.com (http://www.fly-log.com) - a member here made it, I log my flights on it. It eliminates paper. There are also a couple other free online ones, there's even one you can download so that you don't have to go online to access it. I forget the name of it at this point though.

Here's a screenshot of my log. I use Excel, its great for tracking pack performance and keeping up with maintainence. How long has it been since I changed the belt, how many flights are on this motor, etc...
