A little tip that might help shave a few seconds off trying to hit throttle hold.
Take a small piece of nitro fuel tubing and slip it over the switch you use for throttle hold.
This makes it very easy to find by the feel of the rubber tubing.
I use pink tubing which is also very easy to see on my radio.

I like to keep a finger under the flight mode on left and throttle hold on right throughout the flight. It's too late if you have to look or grope for a switch.

That's the nice thing about having the throttle hold switch be the only one on the top of the TX on the right side: I can just sweep my hand across the top of the TX and I'm bound to hit it. No groping necessary.

I hold my DX7 with one finger under the TH switch (top right) and one under the flight mode switch (top left). I usually flick into idle up 1 on the ground after spool up, Ive yet to need to flip back into normal.
Usually when Im heading in (unintentionally or not) Ive already hit TH before it hits the ground. No fumbling required.
other TX's might not have such a good arrangement of switches.

Try holding radio this way. I will feel strange for a while but in the long run controll much better:

By the time you look down and see pink, you can look up and exploding heli.
I saw this tip way back, and even got some short pieces of various colored fuel line tubing (LHS gave me some short pieces for free). Never got around to installing them, and don't miss them.
Like others, one finger under TH ready to flick. Other finger over Flight Mode in case I need to get back into Normal for some odd reason.
