Does anyone know the web link to get an AMA card. I have been told I need to get one. And I can not find it on this site. Maybe I didn't look in the right spot. Thank you

Quickest way is print out the application and fax it to 765-741-0057
Give them a call at 800-435-9262 and they will fax back your confirmation and you will have everything you need.
