I cannot really vote. Why? because it all depends on what I practice when using the simulator.
So there are times where I have just fun messing around and times where I try to safe it even in a sim.

On the sim I always try to fly out of it, if I hit TH, i'm most certainly going to crash, as my sim cable for the DX6i is 4 channel only, so throttle hold gives it maximum negative pitch
I use TH often on the mcpx if I'm lost, as there's mostly no dmg as long as it doesn't fall to the ground with power in the blade (in grass).
I haven't used TH in flight on my mini prôtos yet, nor have I needed to. I fly pretty high atm, so I can be comfortable, and have a good margin for dumb sticks & orientation mistakes.

bail out teaches the brain the quick action it requires to do it in real life.

So if something looks wrong and the heli is about to lawn dart, I do what my brain knows to do and fly out of the situation. Usually works.

After hitting my leg with 450 ....I practice coming straight for myself 2 or 3 times per session to either hit th soon enough or which way to move sticks to get away from me.I think sims should be little more graphic like video games when heli gets too close .because when it really close in real life theres no reset.
