Default Tread looker on contr fork for pitch

Just before I lost my best heli to day. Pitch contr fork for the tail rotor unscrewed and tail control got lost resulting in spinning heli. Managed to land it , low hight saved it from crash. I never had tred looker between the fork and the slide bushing ( PV 0425) , have done that now. Should there be tread looker ? Have any one exp the same?

Have had my 720e for about 4 years now. Did have that happen one in the early stages of this machines life. Weird, mine happened during spool up right before lift off. I did apply some type of thread locker and have not had it happen again. In fact, have not had any problems with this machine and I could not tell you how many hundreds of flights it has logged.

It needs Loctite. I had mine come off once. The copper sleeve has Right Handed Thread, so it won't be self tighten.
